Before we get to it, here are a few terms we need to be familiar with.

  • Chronic Disease Management Programme (CDMP)
    These are Diabetes, Hypertension, Lipid Disorders, Stroke, Asthma, Chronic Obstructive Pulmonary Disease (COPD), Major Depression, Schizophrenia, Dementia, Bipolar Disorder, Osteoarthritis, Benign Prostatic Hyperplasia, Anxiety, Parkinson’s Disease, Nephritis/Nephrosis, Epilepsy, Osteoporosis, Psoriasis and Rheumatoid Arthritis.
  • “Simple” refers to visits for a single chronic condition
  • “Complex” refers to visits for multiple chronic conditions or a single chronic condition with complications
  • For recommended health screening, it refers to screening for diabetes, high blood pressure, high blood cholesterol, colorectal cancer and cervical cancer.

How can I apply for CHAS Card?

Assuming you are eligible for CHAS subsidies, here are the steps to do so.

  • Fill up the CHAS application form (found in any Community Centre or Club (CC), Community Development Council (CDC), Public Hospital or Polyclinic)
  • One can also download the application form here.
  • After which, mail the completed application form to:

Editor’s Note:

  • Family with the same NRIC address only requires one application form to be submitted.
  • Parents or legal guardians will need to give consent by signing on the application form on behalf of their family members who are below 21 years old.
  • For those who are mentally incapacitated, the appointed donee(s) or deputy(s) can sign on their behalf.
  • In the event consent has been provided on behalf of any family member(s), additional supporting documents may be required.
  • By signing up for CHAS, you are giving your consents for data sharing between Ministry of Health (MOH) and its partners.

The whole process is about 15 days upon receiving of the application form.


Where can I use my CHAS Card and enjoy the benefits of it?

The most direct way to know if the GP or dental clinic is participating in the CHAS programme is by looking out for the CHAS sticker that will be displayed at their entrance.

What do I need to bring to a CHAS Clinic?

In order to enjoy the benefit of CHAS subsidies, one simply needs to bring these when they register at the clinic:

  • NRIC
  • Valid CHAS Card (Blue, Orange or Pioneer Generation)

For CHAS Cardholder below 15 years old, a student pass or birth certificate will be required.
